Located within the heart of
fair haven, within the larger New Haven community
that
is home
to Yale and five other area universities,
Mary Wade provides residents and clients with a full range
of services; everything they need to continue to live an active,
independent lifestyle.
Along with the following services, we provide
transportation to area cultural and educational events including
performing arts centers such as the New Haven Symphony Orchestra, the Shubert
Theater and Long Wharf Theatre.
Under the direction of a volunteer
Board of Managers, Mary Wade offers:
• 45 Assisted Living Units
providing assistance with medication, prescription refills, bathing/personal
hygiene,
laundry and housekeeping services, three meals a day and snacks
all included in one fee. The program is licensed and funded by
the State of Connecticut.
• 60-Bed
Skilled Nursing Unit providing skilled nursing services for short
term, chronic illness and end-of-life care.
• The Adult Day Center operates
seven days a week. We provide medical assistance to those living
in the community as well as full hot lunch and snacks. Transportation
is offered for a moderate fee. This is a medical model certified by the
Connecticut Association of Adult Day Centers, Inc. and the State of Connecticut.
• The Garden Room is a specialized Alzheimer’s
Day Program operating seven days a week with activities designed
for the unique challenges of those with memory impairment. Rehabilitation,
recreation and other ancillary services are available.
• Center for Outpatient Rehabilitation (COR).
Provides physical therapy, occupational therapy and speech pathology
to the community. Insurance and Medicare accepted. It is conveniently
located with free parking and transportation available upon request.
• Weekend Transportation to those over 60
years of age in the Greater New Haven Community. Saturdays and
Sundays, 8:00 a.m. to 5:00 p.m. Donations accepted.
• Short-term Respite Care in a residential
setting offering fine dining, enclosed outdoor gardens, planned
activities, supervised environment, medication monitoring and
weekend transportation.
Funding for various programs available through
the Agency on Aging of South Central Connecticut, Connecticut
Department of Education for Nutrition Program, Medicare, Medicaid,
